Playcademy Parent/Guardian Agreement & Notice of Policies

I represent that I am the parent or legal guardian of the child I am enrolling, who is participating in a term, class, program or other event (hereinafter the “Program”) by or with Playcademy. (hereinafter “Playcademy”).

I hereby certify that my child is in good health and that my child has no limitations that with or without a reasonable accommodation would preclude his/her safe participation in the Program.

I hereby give my authorization and approval for my child’s participation in any and all of the activities of the Program. I further acknowledge and agree that participation in the Program involves activities (known as “Class”) in which students participate in activities, move about, and further, that risks include physical injury.

I further acknowledge that the above is not inclusive of all possible risks associated with the program and that the above in no way limits the extent or reach of this release.

By enrolling my child in the Program, I agree to release, indemnify and hold harmless on behalf of myself, my heirs, representatives, executors, administrators, and assigns Playcademy, its officers, agents and employees from any cause of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence, which I my heirs, representatives, executors, administrators, and assigns may now have or have in the future against Playcademy, on account of any personal injury, property damage, death or accident of any kind arising out of or in any way related to my child’s enrollment in the Program.

Through my signature below, or checking the appropriate box during online registration,  I acknowledge that I have read this Agreement and understand that it includes an assumption of the risk and a release of liability. I understand that Playcademy is relying on this waiver in allowing my child to participate in the Program.

I have read and understand the above information and agree to its terms.

After School Registration/Early Termination

By enrolling in Playcademy’s After School Class program I am committing to my child’s participation for the full term, and that my credit card will be charged monthly over the course of the after school term. If I must terminate my child’s after school  participation at any time during the term – or within 30 days of the the term’s starting month – my child’s Playcademy classes will end and I agree to pay the early termination fee. The early termination fee is per term and equal to the regular tuition fee for one month in the term.

After School Make Up Policy

After school is not held on public school holidays. These closures are factored into the term’s overall fee. There are no make up classes offered for child absences.  Playcademy does not issue credits for missed classes.

Lunch/Snack Policy

Any foods brought in must be tree-nut free and not require refrigeration.

I hereby acknowledge that I’ve alerted Playcademy’s ownership of any medical issue, dietary issue, allergy or illness that could impact the safety and well being of my child and her peers in the class. If my child might require an EpiPen, other medical device or medication, I have left it (labeled with my child’s name) with the staff with explicit, written instructions.

Emergency Medical Treatment

I do hereby give permission to Playcademy to obtain the necessary emergency medical treatment for my child, with the understanding that the family is notified as soon as possible. If my child receives an injury during the session, I am responsible for any medical expenses incurred.

Lost and Found

Playcademy recommends that you label all personal items with your child’s name. Playcademy is not responsible for any lost, stolen or misplaced items.

Design Release

I understand that my child will be participating in large, collaborative game design and learning efforts, and acknowledge that any contribution, play-testing feedback or design suggestions offered up as part of the program are property of Playcademy and eligible for use in Class programming, marketing materials and product line.

Photo Release

I permit the program to use photographs of my child and/or her projects and work in program materials. I understand that personal information about my child will never be shared in conjunction with these photographs without my permission.

Other Policies

I understand that Playcademy reserves the right to ask my child to leave the session if it is determined that he/she is in any way disruptive to the Program. Any decision made by Playcademy in this regard will be in Playcademy’s sole discretion. Playcademy asks that the parent/guardian disclose all pertinent information regarding their child’s past school experiences and/or behavior so we may help you decide if our classes are appropriate for your child. If the parent withholds information pertaining to special needs, behavioral, physical or other and it is determined that Playcademy is not appropriate for your child, refunds will not be given. If we find out that your child does not meet the age requirements for the session, your child may be asked to leave and no refund will be made. Similarly, if your child is asked to leave the session for any reason, no refund will be made.
